3.phone number to the recipient of the shipment - necessary if some of them are selected
delivery methods (required by carriers).

If, when placing the order, the Customer chooses to issue a document
sales for data other than those indicated when registering or placing an order, please
additionally to provide:

1.name and surname or name of the company / institution - necessary to issue the ID card
sales;

2.address (street, house number / or house and apartment number, zip code and city) -
necessary to issue a proof of sale;

The data provided when placing the order are processed by paperlookbook.pl i

the registration or ordering process, please only provide your e-mail address at
